Health Care Disparities and Cultural Competency – The Community Health Network of Connecticut Foundation (CHNCT Foundation) proposes a new initiative: Capacity Building for Cultural Competence(CBCC). With the goal of reducing health disparities in Connecticut by increasing the capacity of health care agencies to provide culturally competent care, CHNCT Foundation will provide training and technical assistance to individuals and organizations within its extensive network. |

CHOICES Program – CHOICES is a Nutrition Education Initiative program with CHNCT and the CT Department of Public Health. Our mission is to provide high-quality, culturally appropriate nutrition and health education for children and adults through professional partnerships that function to increase health behaviors, resulting in positive life style changes for participants. We have created a sustainable program that encourages and promotes the integration of healthy nutrition choices into the daily lives of participants - leading to long term, healthy outcomes. KHAIR (pronounced care) – This is a truly unique initiative through a partnership with the Clifford Beers Clinic in New Haven, we are enhancing self-esteem for foster children and others with troubled lives through pairing them with a hair stylist for one year. Khair aims to serve at-risk adolescents by pairing them with hairstylists or barbers who have committed to monthly appointments for one year. The participating salons and barbershops are also providing direction for routine hair maintenance with no charge to the adolescent. Faithfully Fit – Faithfully Fit is a faith-based nutrition initiative that was launched in September 2007. Over a span of 8 weeks, teams of 6 people from 6 Bridgeport churches with active health ministries will participate in a friendly and healthy fitness competition. Our goal for this initiative is to increase awareness about fitness, nutrition and health within the faith-based community. Our mission is to provide the skills, knowledge and activities one needs to maintain optimal physical, spiritual and mental health.
