Through the Grant-Making Program, the Foundation will make grants to tax-exempt Community Health Centers and other tax-exempt organizations that promote health in CT. By making such grants, the Foundation will further its mission of supporting programs that fundamentally improve the health of the people of CT.

 

The Foundation accepts inquiries, grant proposals, and requests from 501(c)(3) tax-exempt charitable organizations. Our grant making is restricted to requests that fit into our initiatives and interest areas. The foundation is open to reviewing and evaluating innovative programs. The Foundation will make grants to organizations that address the following:

  • Connecticut community health centers and other health organizations that foster a healthier community and result in the development of new and/or improved models for quality health care delivery, disease management and disease prevention
  • Long-term solutions that expand access to health care for poor, distressed and underprivileged individuals and families living in CT
  • Improve the quality and delivery of health care services for poor, distressed and underprivileged individuals and families living in CT
  • Advance and promote the health and well being of children in CT
  • Increase health education, and focus on disease management and prevention
  • Focus on behavioral health for women

The Foundation does not accept nor respond to unsolicited inquires, requests and proposals.

 

What we do not fund – The Community Health Network of CT Foundation, inc. seeks to maximize the effectiveness in grant making in the state of Connecticut. Consequently, the Foundation does not fund projects or requests from outside of Connecticut. Additionally, the Foundation does not fund:

  • Proposals or requests for ongoing general operating expenses and existing deficits
  • Direct support of individuals or religious organizations
  • Lobbying activities of any kind
  • Endowments and capital costs
  • Separate budget line items labeled as “indirect or overhead costs”
